On 11.01.16 I visited at AXIS Bank Branch, Yamuna Vihar, Delhi 53 for opening of PPF Account. first of all, Branch Officials told me that this branch does not provide PPF account facility but when i made contact with Branch Head, Khan Mkt. Delhi then I came to know that Axis Branch of Yamuna Vihar maintains PPF Account. After about 2 hrs Branch Manager accepted my request for opening PPF A/c. One Branch official fulfilled my application form for opening of PPF A/c in photocopies form. When after about 15 days I went to Axis Bank, Yamuna Vihar, Delhi, I came into my notice that my application form for opening of PPF A/c has been rejected from Head Branch, Hydrabad due to application form in photocopies form. In the month of Feb. 16, my application was again fulfilled by Branch Official but when on 22.02.16 I again visited at Yamuna Vihar to get status of PPF Account then I again came to know that my application was again got rejected due to error in customer`s name. Moreover, I want to bring into your notice that form of Customer Profile Sheet for Resident Individuals was filled by Mr. Ankit Kumar, Emp. No. 92898, Axis Bank Official, Yamuna Vihar Branch, Delhi in office without visit at my home on 22.02.16 at 01:15 PM in which Mr. Ankit Kumar as above filled my family details i.e. Total No. 5, Working 2, Children 2, Adult 3 which true facts are as total No. 8, Working 4, Children 2, Adult 6. If you analysis CCTV footage of Axis Bank, Yamuna Vihar, presence of Mr. Ankit Kumar Emp. No. 92898 will be found in office on 22.02.16 at 01:15 PM. on 22.02.16 I made objection to Branch Manager on field verification form but Branch Manager did not take any action against Mr. Ankit Kumar after making requests. In the Month of Feb. 16, one day my wife Mr. Ekta Chaudhary visited at the above branch for getting status of my account but neither official informed her about my PPF Account status no official gave Branch customer care no. For opening of PPF Account, I have visited 5-6 times at the above Branch but my PPF account was not got opened in the Branch. Due to not opening of PPF account, I could not submitted my saving in account branch for getting Tax rebate. Now I am bearing loss of Rs. 15, 000/- about can you tell me who is responsible for my loss. It is, therefore, requested that please take strict action against Mr. Ankit Kumar, Emp. No. 92898, Axis Bank, Yamuna Vihar, Delhi and also Branch Manager for favour of all axis bank customers.
