City Bank — credit card application

My name is basidali mahimkar. I am working in forgeahed solutions pvt ltd. I have salary account in citi bank. On 5 august i have applied for city bank credit through your employee nidhi tiwari (Pune kp branch). After 1 week she told me your one signature is pending after that she came to meet me and we complete the signature process. Again after one month she told me your one more signature is needed after that she never came to meet me. I am continuously asking her about credit card application and she is giving me just excuses. Is such service citi bank giving to salary account customers?

Now my question is is my signature is needed then who did my signature. And the banking person itself don’t know where and how much signatures are needed.
