Myself Augustin D'souza x-employee of Andromeda marketing pvt ltd pune 411001(PF no MH/41081/14276 & Employee id:701990).I had applied for PF in my x-company .and according to that they had submitted my EPF form to EPF office(Bandra Mumbai) on 5 Nov 2010.but on 30/05/2011 i got message that your pf is settled and cheque(cheque no:969426 & 936868) has been send to your bank(sanmitra sahakari bank kondwa branch pune).But still the cheque is not received by the bank nor my x-office.so it my humble request to you plz send the cheque to my residential address given below.
