ATM delivery — ATM not received after submission of requisition form

i had lost my AXIS bank ATM few months back and i Blocked the ATM card.I have also applied for new ATM by submitting the requisition form as per the information i got from the bank's receiptionist.when i submitted the form, the concerned person checked everything in details and finally she received it but after the 12 days as told by that person when i went to collect ATM from that branch, she told my form was misplaced or rejected, she was not confirmed whether it was lost or rejected..3rd time i have applied for ATM.And the Agartala AIXIS bank employees are not cooperative and if we ask something to anyone of them, they are not caring any customer.they do not have the customer care mentality...so please do not let them to degrade the good reputation of AXIS bank...becoz i have visited Mumbai branch, kolkata, guwahati, bangalore...all the employees are so cooperative and helpful that i can't express it in words, so do the needful job for the sake of bank's good reputation...
