I have the same problem of gynecomastia and I went for consultation with Dr Ratti.I went to meet Dr. J.B. Ratti on 30th Dec 2014.
On visiting his clinic, I found that it is a small private setup. Nothing like a hospital or a clinic but looked more like a house turned into a makeshift clinic.
My weight and height were recorded and I was made to pay a consultation fee of Rs 500/- beforehand. Please note, no receipt was given to me for this payment of Rs 500/-
Unfortunately Dr. J.B. Ratti was busy in surgery so I met Dr. Neeta Ratti (His wife). I showed her my gynecomastia problem and she immediately told me that yes I do have glands present. She gave me total cost is around 65000rs(55 thousand for surgery, 8 thousand for Jacket and lab charges (around 2 thousand). I was like shocked to see the cost for gynecomastia Surgery.
And please suggest for cost also how can I get back(may be by insurance or something else) because it's large amount.
Now after reading your experiences I have lots of apprehensions and doubts.Please guide me regarding a good doctor from Delhi.

Jb ratti is amazing surgeon. I had read some reviews above about his performance trust me guys he is one of the finest surgeon we have india with huge experience.. he took 58000 total for my surgery and gave me computerised bill. And full details of my surgery and advice on clinic letterhead. You must trust your doctor. He is very busy doctor generally I saw in 4-5 visit that he performs surgery almost daily so there is no question on his work. I had my surgery on 8 September 2015 few days before I can see my improvement.

I got my male breast surgery done by Dr. Dhir in 2014. He did liposuction on both sides and took out glands from the same cut. There was no cut on the nipple. I am very happy with the result and would definitely recommend him.

I have been seeing some of the reviews for few years now but, did not bother to reply. However, I do feel that I must clarify some of the issues that have been troubling some of the esteemed writers. I hope my comments will help in bringing greater clarity to the subject of Gynaecomastia/Male Breast surgery.
1. Gynaecomastia is mainly a cosmetic problem and only very rarely there is an underlying disease element. In some body builders it may be due hormonal supplements.
2. Surgery for gynecomastia is performed 'within the anatomical distribution' of breast. It means that breast surgery is performed only within the breast area only. Areas that are outside the limits of male breasts are not within the ambit of a surgery unless it is specifically requested by the patient. In such a situation, it becomes an 'additional' area which has to be dealt with accordingly. 3. Breast sits on top of pectoral muscle (large chest muscles) and comprises of mainly three components- skin as visible on the outside, mammary glands and fat tissue. In a gynaecomastia surgery, all three components need to be dealt with.
4. Before surgery, the distribution of breast is marked with marker pen. Nowadays, I am also marking the glands distribution by a red pen, as you can see in the attached picture. Glands distribution can be assessed by examination by the surgeon.
5. Axilla (arm pit) is not part of the breast area. Fat deposited on the sides of the chest or under the arm pit is not part of breast area. It is part of the overall obesity. They are mostly seen in obese or overweight persons. Just as there are side tires on the abdomen, similarly there are these fat rolls on the sides of the chest. They can be reduced by liposuction, if specifically requested by a patient. It is usually treated as an additional area.
6. Coming to the surgery for gynaecomastia now: Gynaecomastia surgery or Liposuction surgery by the Tumescent technique has a very high safety profile being bloodless and totally under local anaesthesia. 7. Differences between 'primary surgery' (first time) and 'secondary surgery' (revision or correction surgery):

b) Uniform and adequate reduction of fat should be achieved in the first time. Glands removal is also usually complete.
WHEN INCOMPLETE SURGERY HAS BEEN DONE, What to do?
1. When a patient has undergone a surgery on breast but there are visible irregularities or unevenness, that is, signs of incomplete removal of fat and glands, areas of depression and elevation, the management becomes very difficult. Let me explain the reasons.
a) Excessive fat removal by liposuction will result in depressions and formfirm adhesions between skin and chest muscles directly. These are easily seen on the operated area. These adhesions are fibrous and very firm. It is usually not possible to fill these depressions because of almost complete loss of fat in that area. They can be corrected with either fat transplantation or by putting in an implant, like Gortex. In some patients depressions can be corrected by internal transposition of tissues. Depression in the area of areola (dark circle around the nipple) is usually because of excessive removal of breast plate under the areola. Again, this is also very difficult to treat and needs an external agent to be put in between the areola and muscles.
b) Therefore, it is very important that the primary surgery (first time surgery) should be as good as possible. However, anatomical complications are difficult to treat by any surgeon.
Having said all that, it should be understood that surgery is a skill which is acquired by surgeons after years of experience and learning. No other profession demands as much dedication of time, devotion and sacrifice as medical profession. Complication or deficiencies in results may happen in the hands of best surgeons anywhere in the world. After all, surgeons deal with too many variables when performing an operation. In gynaecomastia surgery, although one may be suffering from male breast problem, yet EVERY PATIENT IS UNIQUE AND DIFFERENT FROM ANOTHER in terms of local anatomy and presentation.
I have personally performed more than 3000 gynaecomastia surgeries, all by tumescent technique, 'awake' surgery, totally under local anesthesia. 99% or more of my patients are very happy because I have seen lives being transformed, confidence restored and careers having taken off to higher level simply because of a successful gynaecomastia surgery. I have gynaecomastia patients who have become my very good friends even though I performed their surgeries 10-15 years before. I cherish their friendship and thank them for having reposed their confidence in me.
1% or less patients may not achieve complete satisfaction which is understandable. No surgeon in the world can claim 100% success always. The reasons for that could be patient's personal issues, unrealistic expectations, unresolved psychiatric problems (with or without treatment), financial difficulties or maybe little less than optimal surgical results on surgeon's part.
A surgeon is motivated only by the single consideration that is, to bring happiness to his patient. It takes decades of experience and continuing studies to achieve those high levels of dexterity. There is no greater reward for a surgeon than to see a smile on his/her patient's face.

I am Dr JB Ratti from VITAL CLINIC. I have not spoken in all these years but, I have been requested by many of my patients to write. They have pointed out some comments about me on this site. There I had detailed the basic concepts of breast anatomy, surgery and resolution of depressions and unevenness from a previous gynaecomastia surgery. I hope it was helpful to all those who may be considering getting a gynaecomastia surgery done. I am writing these lines to clarify few more apprehensions which some of the esteemed writers of comments may have. This concerns the financial aspect of surgery.
Surgery charges of any surgery for that matter, are very individual specific. They depend upon size of the breasts, presence or absence of associated sagging of breasts, the expected technical difficulty in a particular patient and the presence of complications from previous surgery. This assessment is made by a physical examination during a consultation. After examination, charges are quoted for the surgery and for any additional procedure required. After that it is for the patient to make a decision for or against getting the surgery done.
Now the issue of giving a surgery quotation on official letterhead. We don't do that. Before ten years or more we used to give such a quote on request. However, there were some incidents where we had to face a difficult situation because of this. Many persons in government or private jobs have the facility of getting money in advance from their respective organisations if they can produce the likely expense detail from a nursing home, doctor or hospital. It so happened on three four occasions that such a financial statement was taken from our clinic by some unscrupulous persons. Using that, they got money released from their organisation but, used that money for some other purpose. In their explanation they simply gave a letter to their organization that the surgery had been done for their relative but, the clinic/nursing home did not give a proper bill! Later on, their organization sent people to our clinic inquiring us to confirm whether the surgery was done on so and so person or not! We, as surgeons, always want to avoid controversies like this and just want to continue doing our work peacefully. Since then some guidelines were laid down and we strictly adhere to them.
We never give a surgery quote without the surgery having been done. After the surgery, full detailed bill and receipt is provided to patients, as per the charges quoted before surgery, at the time of consultation. It is preposterous to think that any patient can be 'forced' to pay more than the sum quoted before!! In our nearly 25 years of surgical practice at VITAL CLINIC, there has never been a single complaint by anyone on this issue!. We are proud of our transparent policies and will continue to adhere to them.
