I am writing this review for the benefit of anyone suffering from Gynaecomastia (male breast) and is thinking o[censored]ndergoing a surgery for it. I have myself been a patient of gynecomastia and have undergone a surgery (5 years ago). However the doctor who performed this operation on me ( Dr. Anup Dhir , working in Apollo Hospital, New Delhi ) did not do a good job . He did not remove the glandular tissue completely and sucked out more fat (adipose tissue) from one side. This has left a visible asymmetry and puffiness in my pectoral area.

In fact after the surgery when I regained my consciousness, I told him that I could still feel the glandular tissue by my fingers. However he told me that it was my illusion and that he had removed glandular tissue completely from both the breasts. He added that it must be the pipes to remove haematoma (fluid) that I can feel. However that was not the case and even after 5 years I still have that glandular tissue which Dr. Anup Dhir should have removed.

As you know, Gynaecomastia is not a fatal disease but an embarrassing condition. All the time you remain obsessed with your unnaturally protruding pecs ( or tits ). This surgery performed by Dr Anup Dhir did bring lot of psychological relief. With clothes the condition is not that prominent anymore. However the imperfection in surgery ( asymmetry and puffiness ) is clearly visible bare chested.

The lessons I learnt
1) We Indians have this habit of equating Doctor with God. These days doctors are more of businessmen . Never trust your doctor blindly or believe in everything that he tells you. Ask questions and never stop probing till you get a satisfactory answer. If possible, take a second opinion or a third before going ahead with a particular surgeon. Remember, it is better to get a perfect surgery done in the first attempt. The revision surgery costs you more and has less chances of giving the desired results.

2) Always keep a record of the receipts and estimates that the doctor provides you. If he botches up the surgery you can go to a Consumer Court with these proofs or ask him to do a revision surgery free of charge. Make sure that he includes post-operative charges in the estimates.

3) Clearly ask him what results you can expect after he performs the surgery. Though no doctor would do that but do ask him if he can give in writing the result he is promising. See what he has to say.


Having learnt my lessons the harder way, today I visited another cosmetic surgeon based in New Delhi ( Dr. JB Ratti , from Vital Clinic, South Extension, New Delhi ) for consultation on the possibility of a revision surgery. I had done a google search and his website looked impressive ( with all the photos of past patients and glands removed ).

On visiting his clinic, I found that it is a small private setup. Nothing like a hospital or a clinic but looked more like a house turned into a makeshift clinic.

My weight and height were recorded and I was made to pay a consultation fee of Rs 500/- beforehand. Please note, no receipt was given to me for this payment of Rs 500/- ( may be to evade tax or the responsibility)

Conversation with Dr JB Ratti started on a positive note as he examined my chest (very casually though). I explained him what I wanted to achieve through this surgery and my apprehensions. He listened patiently. When I started asking him questions as to how he is going to replenish the excessive fat removed from my previous surgery he could not give a convincing answer. Told me to have faith in the doctor. My next question was if he is going to use the Canula Knife to suck out the tissue or remove the tissue using his scalpel. To this Doctor Sahab became defensive and personal. Called me "Confused" and interfering with the doctor's procedure.

In todays time when we have RTI (right to information) is it a sin for a patient to know what his doctor is going to do to him. These doctors give fancy names to their techniques "like Tumescent Lipoplasty " to impress patients and charge hefty fees but are not ready to explain what it really is and how it is different from the regular procedure (which costs half the price)

In fact his much renowed Tumescent techniques was nothing different from the usual procedure ( other than the fact it was performed under local anesthesia by injecting an anesthetic fluid ). Upon further probing he told me that even in this so called "latest" technique, he would be required to make a Semicircular cut in the areolar region (just like Dr Anup Dhir did 5 years ago ) and poke inside the Canula ( a needle ) from various angles to suck out the fat and tissues.

So what is so special about it that I should expect a better result from my last surgery .
The procedure remains almost the same other than the way of putting me to sleep (or keeping awake)

Honestly speaking, I so desperately wanted to get rid of this Gynaecomastia ( devil's lump ) that I was ready to ignore all these doubts and get along with the surgery. I asked him to fix up the date for the surgery to which he happily complied. As a formality, I asked him to give me a printed estimate of the cost involved in the surgery. To my surprise he categorically refused ( citing the reason that it is against his Company Policy ).

In fact he did not even want to give me the rough paper he had used to quote me a hand written figure of Rs 50 thousand for the surgery (with no break up ) and 7 thousand extra for compression garment and blood test. He insisted that I should note it down in my own hand writing.

Needless to say, he refused to give the cost estimate of the surgery on his Vital Clinic letter head. He said that only the final bill will be issued after the surgery is performed (though a part of the payment was required to be made in advance).

It does not take lot of intelligence to guess what Dr. Ratti was upto. He was playing safe. In case something goes wrong during or after the operation then I ( or my family ) would not have any evidence against this doctor.

I thanked the doctor for his time and left his clinic.

Dr J B Ratti 's (Vital Clinic, New Delhi) opaque business practices failed to inspire confidence in me. It is hard to let anyone cut you open if you cannot trust that person.

However I really cannot comment on his prowess as a cosmetic surgeon. He may be really good for he claimed to have performed 2000 such surgeries in the past and had photos as well to back his claim. However, now after meeting him in person I would think 2000 times before going under his scalpel.

If you have been suffering from this condition called Gynaecomastia ( male breast ) then a good surgery can transform your life. But do not ignore the paper work.

I have the same problem of gynecomastia and I went for consultation with Dr Ratti.I went to meet Dr. J.B. Ratti on 30th Dec 2014.

Unfortunately Dr. J.B. Ratti was busy in surgery so I met Dr. Neeta Ratti (His wife). I showed her my gynecomastia problem and she immediately told me that yes I do have glands present. She gave me total cost is around 65000rs(55 thousand for surgery, 8 thousand for Jacket and lab charges (around 2 thousand). I was like shocked to see the cost for gynecomastia Surgery.

And please suggest for cost also how can I get back(may be by insurance or something else) because it's large amount.

Now after reading your experiences I have lots of apprehensions and doubts.Please guide me regarding a good doctor from Delhi.

Jb ratti is amazing surgeon. I had read some reviews above about his performance trust me guys he is one of the finest surgeon we have india with huge experience.. he took 58000 total for my surgery and gave me computerised bill. And full details of my surgery and advice on clinic letterhead. You must trust your doctor. He is very busy doctor generally I saw in 4-5 visit that he performs surgery almost daily so there is no question on his work. I had my surgery on 8 September 2015 few days before I can see my improvement.
From my side I will highly recommend him
I got my male breast surgery done by Dr. Dhir in 2014. He did liposuction on both sides and took out glands from the same cut. There was no cut on the nipple. I am very happy with the result and would definitely recommend him.

hello everyone i also want to share my story about gynecomastia surgery.i am 26 yr old searching abt the cure and best surgeon in delhi from last few years. sometimes i felt that how i face the surgery and what will be the result after surgery.after so many thoughts i decided to meet the surgeon who is best and do surgery in a reasonable price. i started mail to every surgeon and got estimate of 50k or 60k sometimes more than this. as i don't want to share my prob. wid my family the amount which will pay for surgery hve to arrange by myself. during searching for doctors i come to know about dr jb ratti and mail him to know the cost and he rply me that 50k for surgery and some nominal charges of 8k for test and pressure garment. i request hm to give me some discount but he said that this is lowest quote for surgery.i can't even afford this much also then i get to know about dr pk talwar from delhi in gk2 . i went to him and request him that i m not financially strong so he told me to give 40k only for everything . i met hm on 16 nov 2015 and surgery date ws fixed on 21st nov. he did a great job now after 1week i saw my chest as it become flat .i really thanks to dr pk talwar and my god to give me a new life.

I have been seeing some of the reviews for few years now but, did not bother to reply. However, I do feel that I must clarify some of the issues that have been troubling some of the esteemed writers. I hope my comments will help in bringing greater clarity to the subject of Gynaecomastia/Male Breast surgery.
1. Gynaecomastia is mainly a cosmetic problem and only very rarely there is an underlying disease element. In some body builders it may be due hormonal supplements.
2. Surgery for gynecomastia is performed 'within the anatomical distribution' of breast. It means that breast surgery is performed only within the breast area only. Areas that are outside the limits of male breasts are not within the ambit of a surgery unless it is specifically requested by the patient. In such a situation, it becomes an 'additional' area which has to be dealt with accordingly. 3. Breast sits on top of pectoral muscle (large chest muscles) and comprises of mainly three components- skin as visible on the outside, mammary glands and fat tissue. In a gynaecomastia surgery, all three components need to be dealt with.
4. Before surgery, the distribution of breast is marked with marker pen. Nowadays, I am also marking the glands distribution by a red pen, as you can see in the attached picture. Glands distribution can be assessed by examination by the surgeon.
5. Axilla (arm pit) is not part of the breast area. Fat deposited on the sides of the chest or under the arm pit is not part of breast area. It is part of the overall obesity. They are mostly seen in obese or overweight persons. Just as there are side tires on the abdomen, similarly there are these fat rolls on the sides of the chest. They can be reduced by liposuction, if specifically requested by a patient. It is usually treated as an additional area.
6. Coming to the surgery for gynaecomastia now: Gynaecomastia surgery or Liposuction surgery by the Tumescent technique has a very high safety profile being bloodless and totally under local anaesthesia. 7. Differences between 'primary surgery' (first time) and 'secondary surgery' (revision or correction surgery):
a) There is a well defined anatomy in the breast (mentioned above), therefore liposuction and dissection of mammary glands is within known surgical planes and is usually complete. See this video https://www.youtube.com/watch?v=BBV25LyijLc

b) Uniform and adequate reduction of fat should be achieved in the first time. Glands removal is also usually complete.

WHEN INCOMPLETE SURGERY HAS BEEN DONE, What to do?
1. When a patient has undergone a surgery on breast but there are visible irregularities or unevenness, that is, signs of incomplete removal of fat and glands, areas of depression and elevation, the management becomes very difficult. Let me explain the reasons.
a) Excessive fat removal by liposuction will result in depressions and formfirm adhesions between skin and chest muscles directly. These are easily seen on the operated area. These adhesions are fibrous and very firm. It is usually not possible to fill these depressions because of almost complete loss of fat in that area. They can be corrected with either fat transplantation or by putting in an implant, like Gortex. In some patients depressions can be corrected by internal transposition of tissues. Depression in the area of areola (dark circle around the nipple) is usually because of excessive removal of breast plate under the areola. Again, this is also very difficult to treat and needs an external agent to be put in between the areola and muscles.
b) Therefore, it is very important that the primary surgery (first time surgery) should be as good as possible. However, anatomical complications are difficult to treat by any surgeon.

Having said all that, it should be understood that surgery is a skill which is acquired by surgeons after years of experience and learning. No other profession demands as much dedication of time, devotion and sacrifice as medical profession. Complication or deficiencies in results may happen in the hands of best surgeons anywhere in the world. After all, surgeons deal with too many variables when performing an operation. In gynaecomastia surgery, although one may be suffering from male breast problem, yet EVERY PATIENT IS UNIQUE AND DIFFERENT FROM ANOTHER in terms of local anatomy and presentation.

I have personally performed more than 3000 gynaecomastia surgeries, all by tumescent technique, 'awake' surgery, totally under local anesthesia. 99% or more of my patients are very happy because I have seen lives being transformed, confidence restored and careers having taken off to higher level simply because of a successful gynaecomastia surgery. I have gynaecomastia patients who have become my very good friends even though I performed their surgeries 10-15 years before. I cherish their friendship and thank them for having reposed their confidence in me.

1% or less patients may not achieve complete satisfaction which is understandable. No surgeon in the world can claim 100% success always. The reasons for that could be patient's personal issues, unrealistic expectations, unresolved psychiatric problems (with or without treatment), financial difficulties or maybe little less than optimal surgical results on surgeon's part.

A surgeon is motivated only by the single consideration that is, to bring happiness to his patient. It takes decades of experience and continuing studies to achieve those high levels of dexterity. There is no greater reward for a surgeon than to see a smile on his/her patient's face.
I am Dr JB Ratti from VITAL CLINIC. I have not spoken in all these years but, I have been requested by many of my patients to write. They have pointed out some comments about me on this site. So, I have decided to write these few lines. Hope you will find them useful. I have no ill will against anyone and wish all happiness and peace to anyone who may have some negative views. I am writing this just to help.

You will find my comment as from Cosmetic Surgeon, posted sometime back in this trail of comments. There I had detailed the basic concepts of breast anatomy, surgery and resolution of depressions and unevenness from a previous gynaecomastia surgery. I hope it was helpful to all those who may be considering getting a gynaecomastia surgery done. I am writing these lines to clarify few more apprehensions which some of the esteemed writers of comments may have. This concerns the financial aspect of surgery.

Surgery charges of any surgery for that matter, are very individual specific. They depend upon size of the breasts, presence or absence of associated sagging of breasts, the expected technical difficulty in a particular patient and the presence of complications from previous surgery. This assessment is made by a physical examination during a consultation. After examination, charges are quoted for the surgery and for any additional procedure required. After that it is for the patient to make a decision for or against getting the surgery done.

Now the issue of giving a surgery quotation on official letterhead. We don't do that. Before ten years or more we used to give such a quote on request. However, there were some incidents where we had to face a difficult situation because of this. Many persons in government or private jobs have the facility of getting money in advance from their respective organisations if they can produce the likely expense detail from a nursing home, doctor or hospital. It so happened on three four occasions that such a financial statement was taken from our clinic by some unscrupulous persons. Using that, they got money released from their organisation but, used that money for some other purpose. In their explanation they simply gave a letter to their organization that the surgery had been done for their relative but, the clinic/nursing home did not give a proper bill! Later on, their organization sent people to our clinic inquiring us to confirm whether the surgery was done on so and so person or not! We, as surgeons, always want to avoid controversies like this and just want to continue doing our work peacefully. Since then some guidelines were laid down and we strictly adhere to them.

We never give a surgery quote without the surgery having been done. After the surgery, full detailed bill and receipt is provided to patients, as per the charges quoted before surgery, at the time of consultation. It is preposterous to think that any patient can be 'forced' to pay more than the sum quoted before!! In our nearly 25 years of surgical practice at VITAL CLINIC, there has never been a single complaint by anyone on this issue!. We are proud of our transparent policies and will continue to adhere to them.

Hi i m gaurav aggarwal 31 year old guy from haridwar, yesterday (2 nov 16)my gyncomestia prob opreated by dr ratti, i never think opretion could be so easy as mr ratti done, i was completey active when he is doing my operation dr sahab is very caring. about pain, ask many times am i comfortable .
in other word i can say he is totally change the defination of opreation .
No pain no full body anethisia no big cuts no rest .

If we talking about fees and expenses he is expensive but just like brand and local .mr ratti is a brand for gynomastia surgery i[censored] want a best result chose dr ratti otherwise there are lots of local doc save ur money and go for traditional aperation approch
