Dear Sir,
Myself Ashok Kumar Srivastava, I have invested Amount to Rs 100000.00 (Rs. One lakh) in your company wide folio No GTPL 003850 & Policy No Policy No[protected] Plan PM Code -72 Dated 12 Nov 2011 Agent Code 5949 Mr. Akshaywar Tiwari Mobile No [protected] Branch Manager Mr Rupesh Mishra Mobile No [protected] As per terms and condition of your policy you have to pay me Rs 1500.00 till November 2017 per month in my bank account but you have stopped paying the same. You have not transferred the amount since July 2016 to till date. When I asked the reason to your Agent and your Branch Manager they have not conveyed me the valid reason they told me to talk head office and given me land line number of your office. I talked Mr. Subhashish over land line number He clearly told me that Company will not pay a single coin you can do whatever you want.

When I purchased the policy the Company has issued a postdated cheque No 115410 IDBI Bank Kankurgachi Branch 252, CIT Road Scheme VI M Ground Foor, Kolkata, West Bengal Amount to Rs 105000.00 (Rs. One lakh five thousand only) dated 12/11/2017 and a lot of other false commitment had been done by your Agent Code 5949 Mr. Akshaywar Tiwari and your Branch Manager Mr Rupesh Mishra.
When they had given me the policy papers I told them about the ink which has been used on writing the policy bond and that time they had told me that they will provide another policy bond within a month but they have not provide the same till this date.
I request you to please transfer my unpaid amount of last four months as soon as possible otherwise I will bound to take the appropriate legal action.
Important Point: Please go through attached documents. The attached folia are certified by Deputy Registrar of Companies, West Bengal. You may check the signature of Mr. Anil Mohan Singh (Deputy Registrar of Companies, West Bengal)
