[Resolved]  Piramal Health Care — Non Issue of Family Pension

Dear Sir,
My name is Dinesh Chandra Jaiswal.I was an employee of Rhone Poulenc (I) Ltd which was taken over by Nicholas Piramal (I) Ltd. now known as Piramal Health Care.My Employee No. was 2401 and Pension no. is MH/4505/5808..

I left the company in March,2004.I have submitted form 10D along with form no. 4/5 & form no. 7 in 2005 for settlement of family pension.The same was submitted by Nicholas Piramal (I) Ltd @ the office of Provident Fund Commissioner, Bandra (E) Mumbai on 12/04/2007.Till date nothing has been done in this regard. I am yet to receive my family pension. My colleagues who had left the company after me are getting their family pension every month.

I request you to take necessary action for redressal of my grievances along with my previous dues i.e. from April,2004 onwards.
