I am an ex-employee of M/S prolific systems & technologies pvt ltd, Thane, Maharastra and I have sumitted the withdrawl form of PF first time in last July 2011 in Thane-prolific office and after sometime when i enquired the concerned person in my Thane Prolific office he told he has lost my PF form itself. Second time i sent the form in December 2011. But so far I did not get my claim.Even i enquired in Thane PF office they told they have not received PF form from Thane Prolific Office. My account number is MH / 98120 / 687.
Kindly do the needful to get my claim as soon as possible. awaiting your reply soon.
