Your panchanama for verification has been conducted three yeas back and your good office has not sent the ration card. The address verification process was conducted by Mr B S Patil dated[protected] where the panachama was made by him with two witnesses namely (1) Jayant Krishna Gobole, neighbour and (2) Mr. Kanaiyalal Tapari, neighbour.

I have visited your office ten times where Mr B S Patil was not available and your staff has tried to avoid.

The following informations is hereby asked for your good office.

1) Why ration card is pending from your good office and the your office note in this regard
2) How many rations card are issued after the dt[protected] from your good office
3) Why is ration card is withheld and name of incharge officer since dt[protected] and action taken against them for withholding the same.
