Dear Sir

I have invested in the name of my daughter Suhasini Virbhadra Swami and the Folio numbers are[protected]. I have not received the account statements after 06.05.2005. Please do the needful.

My daugther will attain 18 years on22 March 2009 I love to redeem the Mutual fund. Please advise me the procedures involved.
I appreciate your excellent services which I enjoyed in the past.

Please issue a duplicate Certificate in favour of IPSITA SENGUPTA bearing Folio No.[protected] under the Scheme o[censored]05502 UTI Children's Career Balanced Plan-Growth as the Policy already been Maturied and cannot be claimed due to loss of Certificate.
