Dear Sir/Madam,

I am writing to formally lodge a complaint regarding serious patient-safety and professional-practice concerns that I personally witnessed at the following dental clinic:

My 10-year-old son was undergoing dental treatment at this clinic, including a root canal treatment. During the first sitting of the root canal procedure, I witnessed an incident which caused considerable concern regarding the competence, training and supervision of the person assisting the treating dentist.

On 16th Aug 2026, while the dentist was performing the root canal procedure, a female attendant was assisting with saliva suction. During the procedure, the attendant was handling/pushing the cotton placed inside my son's mouth. Due to the manner in which the cotton was being handled, my son accidentally swallowed the cotton.

This resulted in an immediate panic situation. Since my son is only 10 years old and was undergoing an invasive dental procedure, the incident was extremely distressing for both the child and me as his parent. It also raised serious concerns regarding the basic precautions being followed to prevent foreign materials such as cotton from being accidentally swallowed or aspirated during dental treatment.

Concern regarding the qualification and training of clinical attendants

My concerns do not relate merely to the unfortunate incident itself. I am particularly concerned about the identity, qualifications, training and clinical responsibilities of the persons assisting the dentist.

The female attendant who was assisting during the first root canal sitting on 12th Aug 2026 appeared to be functioning as a dental/nursing assistant during the procedure. However, during the subsequent sitting, I personally observed the same woman cleaning the floors of the clinic.

I also observed another male attendant at the clinic who appeared to be inadequately dressed for a clinical environment and who appeared to have difficulty identifying/handling the dental instruments.

These observations have given me serious and reasonable grounds to question whether the individuals assisting the dentist are appropriately qualified and trained to perform clinical duties during dental procedures.

I want to make it clear that I am not claiming, as an established fact, that these individuals are unqualified. Rather, I am requesting the competent regulatory authority to independently verify their qualifications, training, registration/status, job roles and the nature of the duties they are permitted to perform.

Request for regulatory investigation and surprise inspection

In view of the above, I respectfully request the competent authority to conduct a thorough investigation and audit of Dental Affection.

In particular, I request that the authority consider conducting an unannounced/surprise inspection, without prior notice to the clinic, if permissible under the applicable rules and procedures.

I also request that the identity and personal information of my minor son and myself be kept confidential to the extent permitted under applicable law and procedure.

I sincerely hope that the competent authority will treat this matter as a patient-safety and professional-practice concern and conduct an impartial investigation.
