Dr. Lal Path — Name Difference in online report due his reason blood reports is doubtful this is my or some one others.

Dear Sir,
Today morning at 11.30 your staff of lal path collection centre george Town, Prayagraj collected the blood sample from my home, patient name is Aditya Prakash Srivastava but in online report sent by your site patient name was printed as aditya prakash singh hence its doubtful whether it's my report or someone else.
Plz look it this issue and take the action against them.
