UPTET Passed Certificate — How to get UPTET passed certificate

I am jagbir rana,In 2011 my wife give UPTET exam and passed UPTET.
But due to some personal resion we didn't collect the UPTET passed Certificate from the Utter pradesh madhyamik shiksha parishad.now i want to know how we can get the certificate.
