I'm Rajiv Ranjan Anjaney bearing IP No. [protected]. I was under treatment in ESIC dispensary, Madgao, Goa from[protected] to[protected]. I've already claimed over there to get my benefits, but delayed to submit final certificate (which is submitted in after half of Dec. 2015) to get my benefits, but they hasn't provided me any claim no. When I'm trying to check my claim status, then it asks for claim no which is not available to me. Please help me regarding that, so that I can get my bebefit. And please provide me my claim no.
