Dear Ma’am/Sir,

I am writing to request your urgent assistance regarding the continued mapping of my PAN with the MISP DP (Motor Insurance Service Provider – Designated Person) record.

As per the verification conducted today, my PAN is still showing “Matching Record Found in MISP DP”, even though I have already surrendered/ceased my association with HDFC ERGO.

I have already requested the delinking/removal of my PAN from the HDFC ERGO portal. However, the latest verification still shows a matching record under MISP DP.

Therefore, I kindly request you to:

Remove/delink my PAN from the MISP DP record associated with HDFC ERGO.

Please ensure that there is no active MISP DP/agency/insurance-related record mapped to my PAN from your end.

Kindly update the concerned system/database and confirm once the record has been successfully removed.

Please provide me with a written confirmation/email confirmation of the delinking/removal.

This matter is extremely urgent as the existing PAN mapping is preventing me from completing the joining formalities with my new company.

I request you to kindly treat this matter as URGENT and resolve it at the earliest.
