It is hereby submitted that I am the account holder of your Bank (Ambalappuzha Branch, Alappuzha) having my Savings account No. [protected] and against this account I have drawn chedue book bearings numbers from 149761 to 149780, 403111 to 40315 and unfortunately I have lost all these bunch of cheques and my search become futile. I already filed a complaint before the Ambalappuzha police and Dhanlakshmi bank, Ambalappuzha Branch, but the said branch did not take any action till this days. Out of this I have used the following numbers cheques.
Hence it is requested that no cash shall be disbursed if anyone misuse these cheque leafs and misappropriate and submit it for encashment before your bank.
