Hp Gas (Sri Jagannath Gas Agency, Bhadrak, Odisha) — regarding ujjwal gas connection in my name

Dear sir
I sri santosh ku nayak in form you that my wife smt madhusmita nayak is eligible for ujjwal gas connection. I am in village - miripur, gp-talagopabindha, via-annapal, dist-bhadrak 756116. I ve no gas connection in my name and also my husband name. My sl no - (88210900801220000022310178002). I have applied for the hp gas connection at sri jaganath gas agency, nayabazar, bhadrak on 1st jan 2018.

1st time — error - duplicate gas connection.
2nd time error — madhusmita nayak adress is different.

Now i ve also submited kyc no with all ration card and all testimonial to hp gas. Till time no result.

Plz help me regarding this now i am facxing many problem...
