I Kalpana Sahu the policy holder o[censored]ltimate Grand Protect Plan from Kuzalin Service Private Limited a team of Health Assure. The aforesaid plan was FRAUDLY sold to me FRAUDLY sold to me online via link through may be an agent (Maheshwari Verma contact No [protected], [protected], [protected], [protected] of Mercury Insurance Brokers Private Limited or otherwise). that above agent told me Service Charge and GST on claiming such maturity amount of some plan purchased from ING Vysya company Lt by my demised father and which is totally false and also told me that the form of Policy/plan on 27/02/2024 amounting to Rs 41, 300/- (Rupees forty-one thousand three hundred only).

Please cancel my above policy as I am not satisfied with the terms and condition of subject policy/plan and you are requested to cancel my policy as I did not utilize the any facility of the plan and refund the above amount in my account as soon as possible.
