Maithri Developers — I'm complaining about repayment

I have purchased a policy from maitri suvarnsiddhi pvt ltd branch srikakulam of net value 51, 200. I have paid all installments and now i want to apply for repayment. Agent is also cruel at us. All of our neighbours paid this searched for online form but couldn't find. Please suggest any type of solutions possible. Pls mention website to fill online form asking for repayments. Pls help me. Thank you. I am waiting for your reply...
