Subject: Regularization of my EPF Pension
Dear Sir,
This has reference to our previous letter dated 08.03.2017 and 29.04.17 regarding regularization of my pension order no MP/GWL/00019719. The referred pension has been suddenly stopped to credit my SBI account no [protected] from the month of February 2017 .I would like to inform you that the life certificate / Jeevan parmaan certificate deposited to the State bank of India in the month of December 2016 . The copies of life certificate along with jeevan parnaan also sent on dated 29th April 2017.
You are further requested to kindly regularized my EPF pension for the value of Rs 2164/- in to my Saving Bank Account no [protected]. The Photo copy of life certificate counter signed by branch Manager of SBI is enclosed for regularization of my above said EPF pension vide pension order no MP/GWL/00019719
