I ExTSNK/AA BAMBHANIYA SURSINH JIVABHAI, I have
retierd from 306 Field hospital Pin-903306, C/O 56 APO at 31/01/2013. I
have send you house hold luggege claim documents to office of the Pr.C
of A(Fys), KOLKATA-700001 of Latter No.1843/M-3/2013 at 13 March
2013.My total claim amount is Rs.27934 rupees twenty seven thousand
nine hundred thirty-four.
i have many time phoned to the CDA Kolkatta. they have said me that
they have stoped to claim from 1rst April 2013 and send my house hold
luggage claim documents to PAO(OR) AMC Lucknow.my claim is not
completed since 10 months above. please regard me to how
to take action about my claim.

(1) Sanction of the competent authority.
(2) Certy. of non issue of luggage warran.
(3) Bill and cash receipt in original.
(4) photo copy of certificate of registration of hired vehicle
(5) photo copy of national permit of hired vehicle
(6) photo copy of Driver's License.
(7) photocopy of insurance cover of hired vehicle.
(8) photocopy of registration of own vehicle.
(9) Photocopy of I Order regarding vacation of gov.md. accn in old duty stn.
(10) photocopy of part I Order regarding allotment of Govt md accn
(11) photocopy of retirement order
(12) photocopy of movement order.
(13) photocopy of specimen signature of offrs auth to sign/countersign
the financial documents.
