My father was passed away in 24th September 2020 in scb medical cuttack, it's been 1.5 yrs death certificate not provided. I went to medical to collect information then doctor has given me his number and told me to not come to medical for further inquiry, he assured me to resolve my problem but whenever i called him he is making excuses and asking for 15 more days to solve my problem but he is just wasting time, everytime i make a call he told me i will send the document to municipality corporation but since now no documents has been send .
