I am narendra kumar shukla r/o ballia (U p). I took a health insurance from above co on nov 13, 2017 and valid unto nov 12, 2018. My policy no is p/161111/2018/012130. I did treatment of eye with cost of rs 25000/- on own expenses. I follow all proper channels and regulation as per co norms. I submitted my claim with all valid documents (Claim no : cli/2018/161111/0541721). After three months company reject my claim on base facts.
