Tata-Aig — Insurance without written pernission and order to deduction from my bank ac on the basis of deceptive phone call

My daughter is a teacher of narayanan gr of school at saltlake, her name is shrutulekha roy. She has just joined the school. When she teceived her 1st salary, she found rs 2175.00 has been deducted from heraccount. She enquired about it to jher banker-axis bank. They supplied the above information begged innocense reg her question how such amt was deducted from her account without her written permission. They also gave her information that tata-aig mat have ordered deduction on the basis pf phone call and otp basis.
Now my question is
How a company, without signature of proposer can go for insurance. If someone wrongly attend such otp sent by fraud companies.
Why a reputed bank like axis bank obey such instruction.
Sir, i am quite innocent of such happening, only my daughter's hapless position evoked me take shelterof yours.
Please guide my daughterso that she an receive back that amt and also suitably compensetted for such harrasement.
