Teamlease Services Pvt. Ltd. — Request for Correction of PF/UAN Date of Exit (DOE)

I am S. Jacob Jeyakumar, Employee ID 1710000, and I was employed with TeamLease from 16-Sep-2019 to 15-Nov-2019.

I have noticed that my PF/UAN Service History has an incorrect Date of Exit (DOE).

Therefore, I kindly request you to verify my employment records and correct the Date of Exit (DOE) in my PF/EPFO records from 16-Sep-2019 to 15-Nov-2019, as per my relieving letter.
