UTI MASTER GAIN 92 — Change to a new address and no dividend recieved

Sir, i have purchased 500 share o[censored]TI master gain-92 (unit certificate no.[protected]/5,[protected]/5,[protected]/5,[protected]/5, 30256807 LS/5 in the name of Neelam Jasuja) issued on 1st August 1992 of total amount rupees 5000 but i have not recieved any dividend since last five years approx. Another thing i have changed my address to a new permanent address as H. No. 92, Rajiv Nagar, Mandi Dabwali, Distt. Sirsa, Haryana, Pin: 125104.
