BSNL SIM — sim card rejected

Dear Sir,
I have mistakenly with out my knowledge on what exactly the pin Number and the PUK number were meant for. I Entered wrong info as i wasn't aware of what it was nor any information is given to the user...

Now the SIM is rejected...
Can You Please Give me information and Make it activated ASAP.

bsnl/gsm SIM — Sim Card Rejected

I just switch off my phone and then on again then it shows on display sim card rejected. Then i call to cutumer care but problem remain same.

Respectfully, I beg to state that i am a old user idea my MbNo. is [protected] but still i am getting stop with that due to some my own mistakes my sim is getting loked, it demands for a puk code which i don't know i was also trying to correct it but during that process there is one message got displayed that "Sim Rejected".So what will be the next procedure to correct this Sim.
Even if, it will corrected by using some tips and entering puk code then please provide me.
Please do as early as possible because i feeling so sad when i do this wrong with my sweet mobile now,
