[Resolved]  CBSE — reissue of my class 10 &12 certificates

Sir,
I had lost my both certificates i had passed class 10 in 2004 Nishant Public School Palwal, Faridabad, Haryana .
In this regard i am seeking your help to over come this problem.

Sir,
I had lost my 10th marksheet.I have asked the procedure from school and they said that some application form is needed which could be get through intenet, but i am not able to find it anywhere.Please tell the site or any other location from where it coluld be get.
I need it as early as possible as i have to attach it in my CPT form.
Last date of submitting cpt form is 31st march.
Please help me out.
