Credit Card — Non received of Credit Card,but bill is raising

I have received a Credit Card Statement Ref No 3028 Tea[protected] from your ICICI Bank Credit Cards .But I have never received the Original Card. It is stated "Your card has been blocked due to non payment".Would you PLEASE AARANGE SENDING ME THE CREDIT CARD .EVEN I DO NOT KNOW THE 16 DIGIT CREDIT CARD NO WHICH I REPEAT I DO NOT HAVE RECEIVED THE CREDIT CARD NO EVER. Incidentally some no is given in the following patern (4443xxxxxxxx4005) assumed the Credit Card No is gape.

I am also interested to get the said Credit Card in Original in my hand for which the billing amount is mounting each & every month.Please acknowlodge.
