Respected Sir /Mam, for above mention Policy no we had make the claim for as per Final Diagnosis-Vollar Plate # of Middle Finger with Dislocation of Proximal Inter-phalangeal Joint of Lt Hand, Operation:- Open Reduction of PIP Joint with Vollar Arthroplasty with Fixation of # Bone done by Operative DR -Dr Niraj Vora DNB Ortho Jamanagar, under Brachial Block anesthesia.
As per your Listed Grade for various surgery, this type of Final Diagnosis and Final Operation is not cover in this 4 various Grade, But after claim settlement I have received Cheque of Amount Rs 18000, that means you have considered this big surgery in Grade 1, now I am requesting you that please clarified how you had considered this surgery in Grade 1 even it is not listed in your Gradation.
