[Resolved] IIT Kanpur — Master card of PNB blocked

I was making an online transaction (registering for GRE test) with my PNB debit card which is also a master card. It required providing two passwords. I got confused which one is which and entered wrong ones three times. Due to this my card has been blocked and i cannot make further transactions. IIT Kanpur — spelling mistake in my name

Dear Sir,
In my PAN card no. (AHEPV9096N) my name spellings is not correct. I have submit copy of passport for address proof in which it was mention HARISH CHANDRA VISHWAKARMA. Please do the needful and send me a new PAN card.
