Sub: Transfer of Employee Provident Fund Account from KN/25721/05 to KN/14115/1082 – reg.
Ref:- Your letter No. KN/PF/BN/SEC-15/1039/08, dated 04th November’2008 asking for Form-3A.

This is to inform you that, while sending Form-13 for transfer of EPF account, I have not attached Form-3A for the year’[protected]. Our account person told it is not necessary, but any how I took it from our old company and it is with me only.

My old company person informed me about my return of EPF account from your office. So, I am re-submitting the claim along with original Form-3A for the year’[protected] for transfer of EPF account from KN/25721/05 to KN/14115/1082.

Please look in to matter & do the needful, by transferring my EPF account at the earliest to Bommanahalli, Bangalore-68.
