Sir,
I, wish to submit as under
That, going through the phones from the Agents their Phone Nos, of Gobind khan desai, [protected], Pooja Mehta. [protected], +[protected], [protected], Dilip [protected], they have requested me to prepare the Reliance Life Insurance polices and they have assured me for the Loan of Rs. 15,00,000/- over the polices without interest and without benefits over the polices with the installments of Rs. 12,500/- monthly for 10 years period. As their information known I have prepared two Reliance Life Insurance policy bearing client id of policy life assured: 86054406, Contract No. 50966402, and another policy Contract No. 50966300, and I have paid the premium of Rs. 45,000/- and Rs. 45,000/- total Rs. 90,000/- for two polices.
After receiving the polices I have proposed for the Loan purpose but the concerned authorities are refusing for the loan purpose over the polices.
Therefore, it is requested to cancel the above two polices and refund the amount at an earliest.
