[Resolved]  satya — Adding of surname

I have passed my class-x in 2002 from V.V.M,Ranchi and class-XII in 2004 from same institute.I want to add surname in both certificate and marksheet, my name is Satya Ranjan Satpathy but due to some circumstances in certificates & marksheet it is Satya Ranjan. I have all documents of Satya Ranjan Satpathy please tell me the procedure also please what all documents do i need to submit.I will be higly obliged.plz reply
