[Resolved]  Standard Chartered Bank Credit Card — FORGED POLICY of BAJAJ ALLIANZ LIFE INSURANCE COMPANY AND PAYMENT FROM STANDARD CHARTERED BANK CREDIT CARD

Dear Sir / Madam,
This is to bring under your kind attention and knowledge that myself Dr. P. K. Singh is in receipt of a policy in my name which is forged and I have never desired for any such policy. The details of the same are as under.

Nothing in the policy detail is correct except my name. My signature, photograph, father’s name, nominee’s name and all the other details provided in the policy document are forged.
