Submission of documents for 3 times

I (09231170766) submit already twice all documents & completed form in Tata indicom Office(Ultadanga,Kolkata). My phone is running for about 2.5 years.Further they want documents immediately otherwise connection will be discontinued. Why !

I have also submitted the documents 2 times in 2 weeks in the last month.. but again my ph is disconnected.. I stay in kolkata, and have submitted the documents in ultadanga. My no. was 4yr old. this is really bad. their employees are also very bad. i am planning to switch to other connection...
but i really want and explanation from tata why is this happening..
