Sir,My Certificate No.JWS/O/M/[protected]:memberID:[protected]JWSPL :Policy No.122300/48/11/279: Period:25/3/2010 to 24/3/2011.Patient Name:Prabhavati D.Jani-Senior Citizen;age 78 female. Patient had a accute Urinary Tract infection alongwith sever fewer. and for this reason she was admited in PARIMAL HOSPITAL.MANINAGAR,AHMEDABAD. With the thought it is clearly with the cashless. Her treatment was started and it was for 10 days i.e. from 21/12/2010 to 21/12/2012. The claim amount was of Rs.40455/- and ony 5,000/- was disbursed by Raksha TPA,Baroda. Mentioning the reason,"amount payable restricted to Rs.5000/- for Non-surgical treatment, hence Rs.35,455/- deducted". At the time of taking the policy it was cleared that any disease other than condition No.4 nd 8 is payable. Due to the weather change and heavy cold continuous fewer complaining was there, and as per the doctor's advise she was admitted and above billing was from Parimal Hospital. Insurance is taken not only for accident purpose but also for any event accidently happened. In this case,it is accidently happened there was an urine infection and for this purpose she was admitted. Now by saying Non-surgical diseases is not reasonable. In my policy docate is not nowhere mentioned such emergency case is not payable.
You are therefore, requested to kindly arrange to disburse the amount which is WRONGLY deducted. This is my objection as per the policy issued by you. In the above policy no. and certificate no. I am entitled to get the stailled amount.
