[Resolved]  Ttk Health Card — till now i didn't receive ttk card

sir my name pandu i am working as a relationship manager indiainfoline ltd , last year in my account hdfc bank salary account dedected 1000 Rs , purpouse of ttk health card i resign date of 31/11/2009 ,till date i did not receive any card , sir my mother some health problems last 4 moths my money was gone around 19000 some testing medicines sir kindly refund this amount
