Dear Sir,
I have been taking treatment at your Ahmednagar clinic since last 1.5 years and Have taken annual consultation plan based on the your recommendation.
Last year, there was no doctor available for 3 months & also the clinic denied to courier medicine despite of that i have been charged for annual membership. Also no invoice or billing receipt received for paid amount.
and because of, poor management lapses happened in my treatment.
After all of this, I decided to continue with my treatment & renewed membership as per agreed plan on EMI basis. since i moved other CIty, clinic denied to offer medicine through nearby clinic & DHT session. Also, not ready to share billing receipt for the paid amount. after enquiring, clinic, clinic denying the same and asking me to visit personally to collect the same & not ready to offer any consultation for paid money.
I have already suffered a huge losses & lapses in treatment.
Hence, I hereby request you to refund for my paid amount & expect a corrective action ASAP.
Else, I am forced to escalate the matter to respective governing authority.
