Army number 14603612 W Ex Nk Shishupal Singh Yadav.in my 64 KB echs smart card my date of birth is printed wrong I e 05 Oct 2018.but as per record my actual date of birth is 05 Jan 1969.dou to this mistake I can not receive medical facility from echs polyclinic as well as echs enpenelled hospitals.l am patient of Broncal Asthma and highper tension and require regular treatment.therefore l apply Data change request for correction of my actual date of birth i e 05 Jan 1969 instead of my wrong date of birth i e 05 Oct 2018 on 15 July 2020.so I highly requested to you please kindly verify my actual date of birth as early as possible.Thanks.

Army number 14662833A Ex Hav(MACP NB SUB) PRINCE KJ.in my 64 KB echs smart card reg no-[protected] mariyam 03 Jul 1951 relation is submitted wrongly(daughter).due to this mistake I can not receive medical facility from echs polyclinic as well as echs enpenelled hospitals.my mother is suffering neurosurgery diseases and require regular treatment. Therefore l had tried to Data change request for correction but it was failed .so I highly requested to you please kindly verify my mothers echs card and please amendment relation as mother(mariyam)for the further documentation. I shall ever Thankful for you.
