To The Officer in charge EME Records Secunderabad-500021 Sub : Change in name and issue of fresh PPO in favour of Bijaya Laxmi Das instead of Bijoy Laxmi W/o-Late Basanta Kumar Das, No. JC224064N Ref : My Letter dated 10.08.2015 forwarded through Secretary Zilla Sainik Board, Mission Road, Buxi Bazar, Cuttack vide his letter no 05/AGA/ZSB/CTC/15 dated 14.08.2015 Sir, Most Respectfully I Smt Bijaya Laxmi Das, W/o-Late Basanta Kumar Das, JC224064N state the following few lines for your sympathetic and considerate view on my miserable case. That I am the widow of Late Basanta Kumar Das, No. JC-224064 N, Ex.Nb Sub, At-Potapokhari, Po-Nayabazar, Cuttack. That my husband expired on 15.07.2015. After that I proceeded to bank for drawl of my family pension with all the records of my husband. There I came to know that my name has been misprinted as Bijoy Laxmi instead of Bijaya Laxmi Das in the PPO of my husband and the bank manager refused to draw my family pension. That seeing no path I proceeded to Secretary Zilla Sainik Board, Mission Road, Buxi Bazar, Cuttack and he suggested me to carry out certain formalities and as per the procedure I submitted all the records such as affidavit in original, Photocopy of death certificate i/r/o Basanta Kumar Das, Photocopy of discharge book, Photocopy of PPO, Advertisement in the newspaper, The Sunday Times of India dated 9.8.2015 and Advertisement in the newspaper, The New Sunday Express dated 9.8.2015 to the Zilla Sainik Board Office for taking up the matter with Record Office. Accordingly Secretary Zilla Sainik Board, Mission Road, Buxi Bazar, Cuttack has forwarded all the documents for publication of Part-II Order vide his letter no 05/AGA/ZSB/CTC/15 dated 14.08.2015. Further it is to intimate that I have received the part-II order no 0992/NE-I/NE&PG/001/2015 dated 23.09.2015 but the zilla sainik board cuttack informed that a fresh PPO has to be issued, a copy of which will be endorsed to bank and me upon which I can get my pension from bank but after a lapse of 3 months no PPO has been received by me or by bank. Finding noway I am compelled to lodge this complaint that my case may kindly be considered at the earliest so that I can get my family pension which can overcome my miserable situation consequent upon the death of my husband.
