Employees Provident Fund Organisation — No transfer of pension in bank

Sir, My PF NO. is UP.25509/3. After January 2015 no pension amount is transferred to my bank account (Punjab national bank) Kosi Kalan Branch. I already submitted my alive certificate to above bank. I will send again my alive certificate in November 2015. Kindly look into the matter and do the needful on urgent basis.
