ICICI Bank — received unnecessary call from credit card department

Hi I did recharge to my idea mobile number with 120. But I didn't get any msg or recharge money also below attachment was the detail of the transaction details about 120 rs deduction from my icici account I requesting it to be refunded.

Hi there is a mismatch in the phone number that the company is stored in the database. i have been reaching u people constantly but dint get any response, and even requested for a call back, but no response from ur end.i want an immediate action to be taken on this.

Sir my loan is pending long time ago. Because my vehicle name is other person ND loan is my name. So I face many problems. I want to close my pending loan immediately ND solve my dispute problem.

This is information my credit card approval status is done and verification complete last 15July but today I am calling customer care no he told me my credit card application decline. And last time I am received call he told me sir your credit card where send your office address or home address. I am not understand why my credit card status decline. Sir I am last 6 month trying credit card regarding when my all Doucments is OK why my credit card approval decline. Sir please help me.
