Regarding policy no.[protected], we have received claim from TPA under claim rejection code L.13(Hospitalisation in a hospital less then 10 beds), but SAL hospital is very large
hospital having 300 beds which is mentioned in H.T.F. NO. OF BEDS COLUMN. So se resubmit file for further action.

We have resubmitted file to you on 19.06.2015.We have sent email to you on 15.05.2015,
18.05.2015, 25.08.2015 and 01.09.2015.P.I. hospitalized two times and also claimed for two times.

As matter is very delayed, pl.expediate the matter at the earliest.
