Maruti Suzuki — non receipt of extended warranty for third and fourth year

my maruti ciaz zdi with chasis no.MA3FXEB1S00117391, ENGINE NO. D13A2493519 was delivered on 29.11.2014 against full payment including extended warranty for 3 rd and 4 th year from m/s. kataria automobiles pvt ltd, khokhra ahmedabad.they collected Rs13112 towards extended warranty. but till date i have not received extended warranty . i was in continuous contact with mr.yogesh modh, executive for maruti ciaz, who finally asked me to lodge a conplaint with you.-gollapudi krishna mohan, d-1001, parishkar-1, behind renault show room, khokhra circle, ahmedabad-3800008, [protected]
