Mr. Dharmesh Chokshi, (Designation – BSM code - 046752, work at Reliance Life Insurance office : Swaminarayan Avenue, Nr. AEC Cross Road, Naranpura, Ahmedabad – 380001, Gujarat.) of your Reliance Life Insurance came to our Residence in March 2009.

For Investment of and we paid cash (cash Mr. Dharmesh Chokshi) Rs. 10, 000/- (Ten Thousand Rupees) in March 2009 & Dharmesh Chokshi was not give me policy Primmum Receipt Reliance Life Insurance Super Golden Year Plan. But for his Selfish motive on Promotion he made rong information provide me.

Reliance Life Insurance – Super Golden Years Plan Policy Detail – No Insurance Policy & it’s only Ulip Investment Policy & I have Invetment fund is low risk – low Return. why surrender value of the policy as on that day is Zero – Give full Issue Date Policy Statement Rs. 5, 000 /- to Till date. What Zero Value What Charge Less in my without Insurance Policy.

In 31, March 2012, we approached for complete withdrawal of our investment policy but Mr. Dharmesh Chokshi (Designation - BSM work at Reliance Life Insurance office : B/104 & 105, Swaminarayan Avenue, Near AEC Cross Road, Naranpura, Ahmedabad – 380001, Gujarat.) Mr. Dharmesh Chokshi was Informed us do not withdraw complete investment policy and informed to wait upto Apirl 2013 with full interest on our investment but till date. We have not heard anything from Mr. Dharmesh Chokshi not from company in writing.


After Maturity of Investment, It is mandatory of the compney inform policy withdrawal information.

In view of above facts and circumstances, we request you to kindly pay investment policy with March 2012 to till date at maximum Interest rate on the above Foreclosure RLIC policy earliest.

Futher, due to false information, promotion etc of Mr. Dharmesh Chokshi, we suffered a great financial loss which may kindly be recovered from Mr. Dharmesh Chokshi and appropriation action may be taken.

We may be informed in writing urgently.
