Tata Sky — Change of name as wrongfully mentgioned by the TV shopkeeper

I have purchased a new tatasky connection through my TV showroom and my Tatasky ID No.[protected]. Instead of my name viz. JANAKI BALLAV MOHANTY, QRS. NO. 06-C, TYPE-IV, CENTRAL GOVT. QUARTERS, DOVERLANE EXTN., KOLKATA-29, it appears that there is some othername is feeded in the above ID No. and the matter was also pointed out to tatasky through phone as well as to the concerned shop keeper, but till date no action has been taken. Pl correct name at the earliest.
