Ajay Kumar — Request for removal of Agency Code linked with my PAN from IRDAI database

Respected Sir/Madam,

I, Ajay Kumar, was appointed as an Insurance Advisor with IndiaFirst Life Insurance Company Ltd. under the following details:

PAN: IQWPK8625P
Agency Code: IFIF1007710
Date of Appointment: 27 September 2023
Agency Status: Resigned
Date of Resignation: 17 November 2023

I respectfully submit that my agency has already been accepted as Resigned. However, my PAN Card is still linked with the above Agency Code in the IRDAI PAN Lookup/Agent Database, due to which I am facing difficulties in pursuing other employment and professional opportunities.

I therefore request you to kindly:

Remove/de-link my PAN No. IQWPK8625P from the above Agency Code in the IRDAI database.
Update the records with IRDAI immediately.
Issue/confirm my Cessation Certificate (Form I-C), if not already issued.
Provide me written confirmation after completion of the above process.
