Bharat Sanchar Nigam [BSNL] — bill amount debited from bank but no response from bsnl about payment receipt

I have my bb monthly bill amount rs. 1466 on 26th march 2018. Amount has been cleared from back successfully. However, no response from bsnl about receipt of money. Further is very frequent from bsnl. Again i don't receive any monthly bill; only get through sms. If it continues i will have to think differently.
The responsibile officer can take action to rectify the shortcomings, i would be grateful to bsnl.

I here by Mallikarjun S Tatti, Lost my mobile phone with the BSNL sim card in bangalore on 19/03/2018 with number : [protected]. I want get a new sim card and hence require an complaint form. Please provide the complaint form to submit in police station so that i can get it signed and produce to BSNL office.
