Cognizant Technology Solution — Provident Fund UAN number not shared to employee

Hi,
I am an employee of Cognizant from 18th Feb 2005 to 9th Oct 2015 with associate ID 122528. I am able to see the provident fund account number in Cognizant payslips. But unfortunately Cognizant does not share UAN number of the PF portal to employees. I need this number to transfer my PF to my present employer. Thank you. Please send the UAN number of my provident fund account to [protected] and link this number to the same UAN number. Thank you.
